摘要：目的：观察上睑脂肪填充量的精确测定公式在自体脂肪填充上睑凹陷中的应用效果 。 方法：2014 年 2 月-2019 年 4 月 ， 术前利用填充公式计算上睑脂肪填充量 ， 行自体脂肪填充上睑凹陷的患者 282 例 ， 观察治疗前后患者满意度、上睑饱满度并记录术后二次充填比例 。 结果：术后随访 3～24 个月 ，失访 7 例 。 患者对术后效果满意 ， 269 例上睑凹陷明显改善 ， 仅 6 例要求二次注射 。 随访患者均未发生感染、血肿、脂肪液化坏死、栓塞、硬结、局部皮肤不平整、一过性上睑下垂等并发症 。 结论：通过术前对上睑凹陷所需脂肪量的精确测定 ， 能够为上睑凹陷提供精准明确的注射量 ， 能有效避免因注射量过多过少影响术后效果 ， 提高患者满意度 ， 值得大家借鉴参考 。

[Abstract]Objective：To observe the application effect of the accurate measurement formula of upper eyelid fat filling amount in the filling of upper eyelid depression with autologous fat.Methods：from February 2014 to April 2019 ， the filling amount of upper eyelid fat was calculated by filling formula before operation.282 patients with upper eyelid depression were filled with autologous fat.The patient satisfaction and upper eyelid fullness were observed before and after treatment ， and the secondary filling proportion after operation was recorded.Results：the patients were followed up for 3 ~ 24 months ， and 7 cases were lost.The patients were satisfied with the postoperative effect.269 cases of upper eyelid depression were significantly improved ， and only 6 cases required secondary injection.There were no complications such as infection ， hematoma ， fat liquefaction and necrosis ， embolism ， induration ， local skin irregularity ， transient ptosis and so on.Conclusion：the accurate determination of the amount of fat required for the upper eyelid depression before operation can provide an accurate and clear injection amount for the upper eyelid depression ， effectively avoid the influence of too much or too little injection amount on the postoperative effect ， and improve patient satisfaction ， which is worthy of reference.

面部衰老是多因素、多层次、多种组织的退行性改变[1] ， 其中上睑凹陷出现较早 ， 是面部衰老的主要特征 。 上睑凹陷常常与上睑皮肤松弛下垂同时发生 ， 主要原因有[2]：先天眶周软组织发育不良、眼部各组织结构萎缩和眼部外伤、手术及眼部疾病所致的软组织缺损 。 轻度上睑凹陷仅表现为上睑外观不饱满 ， 对外观并无太大影响 ， 无需行手术治疗 ，而中重度上睑凹陷对上睑美学有不同程度的负面影响 ， 可行手术治疗 ，恢复美观[3] 。 目前对上睑凹陷的手术治疗方法主要包括：组织的重新定位；注射及填充；以及前两种方法的联合应用[4] 。 其中 ， 自体脂肪填充上睑具有创伤小 ， 恢复快 ， 术后上睑形态自然且材料来源广泛等优点 ，被广泛应用于临床[5] 。 然而 ， 不同程度的上睑凹陷 ， 填充的脂肪量是不一样的 ， 个体术后脂肪存活率的差异性 ， 更使得填充剂量很难把握 ， 为解决以上问题 ， 笔者总结了自体脂肪填充上睑凹陷的诊治经验 ， 并参照国际国内有关上睑凹陷脂肪填充的文献资料 ， 通过建立数学模型精确计算术前脂肪填充量 ， 并运用于临床 ， 取得较好的临床效果 ， 现报道如下 。
1 资料和方法
1.1 一般资料：2014 年 2 月-2019 年 4 月选取行自体脂肪填充上睑凹陷的患者 282 例（564 只患眼） ， 年龄 24～58 岁 ， 上睑有不同程度凹陷 ， 术前均利用填充公式确定上睑脂肪填充量 。
